Bariatric surgery may not be right for everyone, but people over the age of 18 may be considered if they: 

• Have a body mass index (BMI) of 40 or more
• Have a BMI of 35 or above, accompanied with weight-related medical conditions, such as type 2 diabetes, high blood pressure, sleep apnea, heart conditions or joint problems
• Are Horizon insurance patients with a BMI of 30 – 34.9 and a diagnosis of Type 2 diabetes who are receiving medication treatment now qualify for surgery.
